Adolescent PRP Services

Psychiatric Rehabilitation Program (PRP)

PRP services are provided in the setting most conducive to enhancing each youth’s own healthy participation in the community. This may include the youth’s home, foster care home, our office, or appropriate community sites. The intensity of services will vary depending upon the youth’s
individual needs, environment, or diagnosis.

Our Goals for Adolescent PRP

With the input of the client and family, the PUSH Health and Wellness, LLC Adolescent PRP establishes specific goals relating to the areas of personal care, behavioral management, social skills, and other skills that will lead the client and his/her family to a more positive future independent of or with less intensive mental health services support. 

 

Involvement in the Adolescent Psychiatric Rehabilitation Program is voluntary and family participation is vital in nearly all cases.

ELIGIBILTY

* Must be under 16 years of age.
* Must have Maryland Medicaid or be Medicaid eligible.
* Have a documented history of behavioral and/or emotional challenges.
* Must be actively engaged in age appropriate psychotherapy.
* Parent or guardian must be an active participant in services with the client.

REFERAL PROCESS

Step 1. – The client must be referred by a licensed therapist (to include LCSW-C, LMSW, LCPC , or LCADC) who provides care for the client.
Step 2. – Our Rehabilitation Specialist will conduct a biopsychosocial assessment as well as a daily living assessment.
Step 3. – With input from the client, the Rehabilitation Specialist develop an individualized treatment plan based on the clients diagnosis and data collected during the assessment.
Step 4. – A highly trained Care Coordinator will be assigned to the client within 48 hours and sessions will be scheduled.
