Psychiatric Rehabilitation Program (PRP)
The PUSH Health and Wellness, LLC Adult Psychiatric Rehabilitation Program (PRP) is a mental health recovery support program designed as an alternative to and prevention of inpatient psychiatric hospitalization.
Our PRP program addresses the psychiatric needs of the clients with a chronic mental illness. This population requires life skills development and moderate to intensive support to function independently in the community.
The Goals of PRP
The PUSH rehabilitation approach allows adult clients to identify self-directed goals and develop a plan on how to reach their goals. The clinical team is comprised of a Program Director, Licensed Rehabilitation Specialists, and Care Coordinators. Our clinical team ensures a consistent, safe and therapeutic environment for each client. Those participating in the Adult Psychiatric Rehabilitation program will receive but not limited to:
* At least 6 sessions per month focused on goals outlined in the individualized rehabilitation plan.
* Assistance with addressing entitlements.
* Assistance with obtaining gainful employment.
* Assistance with pursuing stable housing
* Life skills training and support
* Individual Case Management
ELIGIBILTY
Must be at least 16 years of age.
Must have Maryland Medicaid or be Medicaid eligible.
Must have a priority population diagnosis (Schizophrenia , Bi-Polar disorder, or Major Depressive
Disorder)
Must be actively engaged in psychotherapy.
REFERAL PROCESS
Step 1. – The client must be referred by a licensed therapist (to include LCSW-C, LMSW, LCPC , or LCADC) who provides care for the client.
Step 2.- Our Rehabilitation Specialist will conduct a biopsychosocial assessment as well as a daily living assessment.
Step 3.- With input from the client, the Rehabilitation Specialist develop an individualized treatment plan based on the clients diagnosis and data collected during the assessment.
Step 4.- A highly trained Care Coordinator will be assigned to the client within 48 hours and sessions will be scheduled.
